Stem Cell Hair Transplant & The Future of Hair Restoration

    What Is a Stem Cell Hair Transplant?

    Stem cell hair transplants are a group of experimental treatments aimed at getting non-productive hair follicles to produce new hair.

    A typical process involves taking a small amount of fat, bone marrow, or scalp tissue. The cells in these tissues are separated to isolate those that have a special ability to help other cells replenish and regrow, called stem cells. Those cells or substances released by them are then injected into areas affected by hair loss.1

    These regenerative approaches are being studied to see whether the specialised cells can send growth signals to hair follicles that have almost disappeared (miniaturised) and get them to resume normal growth patterns, or perhaps even one day generate new follicles entirely. 

    Why It’s Being Considered 

    The major limitation of hair transplantation is the availability of hair that can be taken from one part of the scalp to cover another. 

    Stem cell hair transplants have the potential to get around this limitation by allowing follicles to be either repaired or multiplied. It is hoped that this type of treatment may eventually help people with extensive hair loss and a poor donor density where hair transplant surgery cannot be performed.

    Types of Regenerative Hair Loss Treatments

    There are different types of regenerative hair loss treatments (so-called stem-cell hair transplants).

    Adipose-derived stem cells (from fat) 

    Fat is a highly studied source of stem cells in this experimental field. A small amount of fat is collected by liposuction and is then processed to isolate fat-derived stem cells. 

    There is mixed evidence on this so far, with only a small number of studies being carried out, each with only a small number of participants. 

    The results from these trials showed an improvement in hair density and diameter from the starting point. 2

    Bone marrow-derived cells 

    Bone marrow contains cells that have long been known to have regenerative potential. A study of these cells in people who had immune-mediated hair loss (alopecia areata) or pattern hair loss showed a significant improvement in hair condition at six months of treatment.3

    Taking these cells from the bone marrow is an invasive procedure, however, and has its own set of risks, including bleeding, infection, and persistent pain. This isn’t very practical for a routine cosmetic treatment, especially when the evidence for its use is not yet at a level that would justify these harms. 

    Follicle-multiplication 

    The aim of follicle-multiplication is to take a small number of healthy follicles, expand their number in a laboratory, and then implant them into the scalp so that they can rejuvenate miniaturised follicles or possibly help create new follicles.

    This kind of treatment is in its early stages of development and hasn’t been tested in humans. So far, it has been shown to help form new follicles when studied on human grafted skin on animals. This evidence does not necessarily translate directly into humans, and we need to research further before it becomes an accepted treatment option.4

    PRP and Exosomes

    Platelet-rich plasma (PRP) and exosomes are related treatments but are not technically stem cell-based. 

    Platelet-rich plasma is made from a specific part of the blood (called platelets) that is rich in cell fragments useful for clotting. People who used this treatment and had pattern hair loss showed an increase in hair density in clinical trials.5

    Exosomes are tiny structures containing genetic material with some proteins and fats. They are not complete cells in their own right and cannot reproduce themselves as stem cells can. They are receiving a lot of attention but are still experimental and cannot currently be recommended as a valid treatment option. There have been warnings from the United States Food and Drug Administration (FDA) about the potential of unapproved exosome products to cause serious side effects.6

    Regulatory Status and Warnings

    Stem cell transplantation as a treatment for hair loss currently hasn’t been approved by the FDA in the United States, nor do any stem cell-based treatments for hair loss have authorisation for use in the UK by the Medicines and Healthcare products Regulatory Agency.7,8

    • United States (FDA): The FDA has a published list of approved cellular and gene therapy products. They state that the FDA would need to oversee a clinical trial of these products, including fat-derived stromal vascular fraction, umbilical-cord products, amniotic fluid, and exosomes, before they get approval.
    • UK (MHRA):  Stem-cell medicines have not currently been licensed for hair loss in the UK.8
    • EU / other countries: The European Medicines Agency has issued a warning that unregulated cell-based therapies may have little to no evidence of benefit and can be risky given the differences in their composition and storage.9

    If a clinic is currently offering these treatments, then you should question them. Here is a guide about reasonable questions to ask:

    • What is the exact product name, source, and batch number?
    • Is it made from your own cells or donor tissue?
    • What does it contain?
    • Does it have authorisation for use in hair loss?
    • Is it being offered as part of a carefully planned trial?
    • What objective measurements will be taken before and after treatment, and how long is follow-up?
    • Are repeat sessions expected, and what is the total likely cost?

    The Risks and Unknowns

    Examples of risks of stem cell hair transplants are pain, bleeding, and infection at the collection or injection site. Bone marrow stem cell collection has added risks of infection and ongoing pain after the cells are removed from the bone.1 There is also a risk of contamination of the collected cells from the time when they are collected to when they are injected into your scalp.

Treatments like Finasteride and Minoxidil are proven to help with hair loss and I would advise starting them if you are eligible, as they can help improve your hair  and prevent your hair loss whilst you wait for something that may never come.”

    The long-term safety of these approaches has not been established yet. The FDA, for example, has received reports of infection, unwanted immune or inflammatory reactions, and tumor formation from unapproved regenerative medicine products more generally. These are not specific to hair loss treatment, but are a reminder to understand the risks involved.

    You may not receive the treatment that the name implies. The actual product being sold as stem-cell injections can end up containing few, if any, stem cells inside.10

    Another risk is that you decide to undergo stem cell hair transplant treatment instead 

    of using already established treatment options. Stabilising your hair loss with existing treatments is already proven to work, whereas experimental treatments are unknown.

    You may also not have pattern hair loss and end up being treated for the wrong condition. You should seek a professional assessment of your hair condition by a GP or dermatologist before you opt for such a procedure. 

    Stem cell hair transplant cost

    There is no standard, approved stem cell hair transplant. Costs may vary greatly, as will the actual treatments themselves. In the United States, for example, treatments often run into the equivalent of thousands or possibly tens of thousands of pounds.11 These figures are not directly comparable to the UK, but it shows they can be a large financial undertaking.

    What’s the Success Rate?

    There is no validated success rate for a stem cell hair transplant. There are distinct treatment methods grouped under this umbrella term, and success looks different to each individual. Most studies have also been carried out on people who have non-scarring forms of hair loss with their hair follicles still intact. 

    Conclusion

    Stem cell hair restoration is a growing area of research. No commercial stem cell hair transplant is currently available. The most promising work is in the area of multiplication of hair follicles.2,4 For the moment, stem cell hair transplants are inconsistent, and their long-term safety and success rate are uncertain.

    If you are concerned about your hair loss, getting an accurate diagnosis by attending your GP or a dermatologist is the next best step to take.
